He/she does not demonstrate any signs of being a potential risk to the church. I authorize the above-named child to assist in the VBS Ministry at Calvary Chapel Tustin. I hereby release Calvary Chapel Tustin from any and all liability associated with my child assisting/helping at the church. Parental Authorization
